What is Sculptra and how does it work?

Sculptra is a French-origin injectable biostimulator whose active ingredient is poly-L-lactic acid (PLLA) — a biodegradable synthetic compound also used in medicine in surgical sutures and implants. Unlike hyaluronic acid, Sculptra does not fill space directly, but triggers a controlled inflammatory response that stimulates fibroblasts to produce new type I collagen (the same collagen that young skin produces).

The result is gradual: there is immediate volume after the treatment (because the product is diluted with sterile water), but this is absorbed within 1–2 days. Collagen production begins within 2–6 weeks and continues for 6–12 months, making the skin firmer and more elastic and the tissue fuller. The final result appears within 4–6 months and lasts 18–24 months, and in some patients up to 3 years.

Sculptra suits the "structural restoration" of the face — rebuilding lost volume (cheekbones, temples, jaw angle) and firming the skin overall. It is also used in body treatments: lax upper arms, the décolleté, the buttocks. On body areas Sculptra is especially effective at improving skin elasticity.

Sculptra prices and how the treatment works in Estonia

The price of Sculptra depends on the number of vials used and on the clinic. One vial usually covers half of the face or a smaller area; a full face often needs 2 vials in a single session. A typical 3-session course (one vial per session, 4–6 weeks apart) is the standard treatment plan. The specific price becomes clear at the consultation, after an individual assessment.

A typical visit starts with a consultation in which the doctor assesses the structure of the face, the patterns of ageing (where volume has been lost, what has become lax) and the patient's wishes. Sculptra is not suitable for filling wrinkles directly — hyaluronic acid is chosen for that. The consultation covers why Sculptra is the right choice in this particular case, or whether it should be combined with another product.

The treatment itself takes 30–60 minutes. After the skin is cleansed, a topical anaesthetic cream is applied (15–20 minutes). Sculptra is injected deep using a specific technique (subcutaneously, supraperiosteally) — this is technically more demanding than injecting hyaluronic acid. Experienced clinics such as Health Clinic in Tallinn, ProAge Ilukliinik in Tartu and Dr. Mari Laasma Ilukliinik use doctors with specific Sculptra training. Afterwards it is important to massage the treated area for 5 minutes 5 times a day over 5 days, so that the product spreads evenly — this is the patient's own responsibility.

How to choose a doctor to inject Sculptra

The injection technique for Sculptra is considerably more demanding than for hyaluronic acid — with the wrong technique, granulomas, uneven distribution or palpable nodules under the skin can develop. Be sure to check that the doctor has completed the specific Sculptra training (Galderma, which manufactures Sculptra, certifies doctors). Ask directly how many Sculptra treatments the doctor has performed — this is not a procedure that can be "picked up" over two weekends of training.

At the consultation the doctor must assess whether Sculptra is the right solution for this patient's concern. Sculptra is not suitable in every case — hyaluronic acid is chosen for fine lines, and where there is major volume loss the doctor may combine several products. A good doctor talks through the alternatives and does not offer Sculptra as a "universal solution".

Favour a clinic with a clear price list, where the original packaging is shown (Sculptra must be a genuine Galderma product), with a written consent form and a structured aftercare plan. Because the result of Sculptra emerges over months, it matters that the clinic offers free follow-up visits at which the doctor assesses the response to treatment and adjusts the ongoing plan if needed.

Risks, side effects and contraindications

The side effects of Sculptra fall into common ones (swelling, mild bruising, tenderness at the injection site — resolving within 1–7 days) and more serious ones. The latter include granulomas (small nodules under the skin — these often respond to corticosteroids or need to be removed), uneven distribution, facial asymmetry, over-stimulation of collagen (too much product in one area) and, extremely rarely, infection. The risk of granulomas is lower when the doctor injects at the correct depth and the patient massages the area properly over 5 days.

Contraindications are pregnancy and breastfeeding, allergy to the components of the product, autoimmune diseases in an active phase (especially those that create a tendency towards granulomas), a history of hypertrophic or keloid scarring, and active skin infections in the injection area. Medications (corticosteroids, immunosuppressants) must be discussed separately with the doctor.

After Sculptra injections, avoid the sauna, intense exercise, alcohol and make-up on the injection sites for 24 hours. Massaging the treated area over 5 days (5 minutes 5 times a day) is the patient's responsibility and is important for an even result. The final result appears within 4–6 months, and in the intervening months the shape may change slightly.

Galderma-certified doctors with greater experience of Sculptra treatments also practise in the capital. The wait for a consultation appointment is usually 1–4 weeks.

In Tartu, Sculptra treatments are offered by ProAge Ilukliinik and Medical Aesthetics. Aesthetic clinics in Pärnu, Narva and other county centres often cover basic hyaluronic acid work, but the requirement for specific Sculptra training means this product is available only at some clinics. Always check the doctor's Sculptra-specific certificate. Since the course requires 3 visits 4–6 weeks apart, plus follow-up visits over several months, choose a clinic that is geographically convenient.
