HIFU, SMAS and RF — face lifting without injections

HIFU, SMAS and RF are modern face-tightening methods that make a „mini-facelift“ possible without a scalpel. In Estonia their popularity has risen sharply over the past 3 years — as of 2026, more than 30% of quality beauty clinics offer one or more of them. In this guide we explain the differences, when each is right, and how to choose.

How these technologies work

HIFU (High-Intensity Focused Ultrasound)

HIFU sends focused ultrasound waves to a specific depth of the skin (1.5 mm, 3 mm or 4.5 mm). At that point the device creates pinpoint heating (~65 °C) that „contracts“ the existing collagen fibres and stimulates the production of new ones. The skin surface stays untouched.

SMAS (Ultherapy, Ulfit-X)

SMAS is a further development of HIFU that works deeper — at the level of the SMAS layer (superficial musculo-aponeurotic system). This is the same layer that surgeons cut in a classic facelift. The result: stronger tightening, a longer-lasting effect.

RF (Radiofrequency)

RF sends a radiofrequency current into the inner layer of the skin. The skin heats to 40–42 °C, which stimulates the reorganisation of collagen and the production of new collagen. Devices: Thermage, Morpheus8 RF (combined with microneedling), Venus Versa.

Who each is for

• HIFU or SMAS:
  • A clearly sagging jawline and neck.
  • Age 35+, when collagen loss is clearly visible.
  • Skin tone still intact, but a „slipped“ contour.
• RF:
  • Fine lines and uneven texture.
  • Mild loose skin, age 30–45.
  • A first face-tightening treatment (a less aggressive introduction).
• Combination: a HIFU/SMAS + RF series is the top choice for 35+ clients who want the maximum result.

• HIFU full face + neck: 350–700 € one-off.
• HIFU full face: 250–500 €.
• HIFU neck separately: 180–350 €.
• SMAS (Ultherapy) full face: 550–950 €.
• RF Thermage: 300–600 € for a single visit.
• Morpheus8 RF microneedling: 250–450 € per visit, series of 4×.
• Venus Versa RF: 100–200 € per visit, series of 6×.

Who this is not for

• The very young (under 30, if there is no specific problem).
• Pregnant and breastfeeding women.
• Patients with a pacemaker (a contraindication for RF).
• Active skin infection.
• Very thin skin or isotretinoin treatment (in the last 6 months).
• Those with autoimmune diseases — consult a doctor.

Before the treatment

• 2 weeks before: no botox or filler injections.
• 1 week before: no peels or retinol.
• 24 h before: no alcohol.
• Come without makeup.

The treatment and recovery

• HIFU: 45–75 min. Numbing cream (light). Mild swelling or redness may appear for 24 h.
• SMAS: 60–90 min. More uncomfortable than HIFU — experienced clients use numbing cream + a nerve-calming medication.
• RF: 30–60 min. Very comfortable, a „warm massage“ feel. You can return to your routine right away.

Where it's best to start — recommendations

If you have never had a tightening treatment before, we recommend this order:

1. Start with an RF series (4–6 visits) — you test the technology and the result.
2. If 3–6 months later you need a stronger result, try HIFU once.
3. If SMAS is needed (a clearly „lost“ contour), choose an experienced doctor-beautician.
