Microneedling and mesotherapy — what's the difference

Microneedling and mesotherapy have grown sharply in popularity at Estonian beauty clinics in recent years. The reason: the results are visible, the risks low and the procedure does not require a long recovery. In this guide we explain the difference, who each suits and what to expect.

Microneedling — how it works

A Dermapen or Microneedling RF device creates thousands of microscopic channels in the skin (0,25–2,5 mm deep). The body reacts to this as a micro-injury and triggers a regeneration process: the production of new collagen and elastin fibres. The result: skin structure, tone, pore size and the appearance of scars all improve.

Two main devices in Estonia:

  • Dermapen (mechanical): classic microneedling. Suited to acne scars, pores and fine lines.
  • Microneedling RF (Vivace, Morpheus8, INTRAcel): combines microneedling with radiofrequency. It penetrates deeper and the results last longer. The price is 30–50% higher.

Mesotherapy — how it works

Mesotherapy is the injection of a cocktail directly into the skin with fine needles. The cocktail's composition is tailored to the goal:

  • Hyaluronic acid: hydration, smooth skin. The most common.
  • Peptides and growth factors: anti-aging, collagen.
  • Vitamins (A, B, C, E): antioxidants, glow.
  • PRP (platelet-rich plasma): the client's own blood. The “vampire facial”. Effective for scars.

Who microneedling / mesotherapy suits

  • Acne marks, scars (atrophic), wide pores.
  • Fine wrinkles, uneven texture.
  • Hyperpigmentation, dullness.
  • Thinning hair (specialised mesotherapy on the scalp).
  • A need for skin hydration.

Combining with peels and other treatments

Experienced beauticians often combine:

  • Peel + microneedling: 2 weeks apart. The peel prepares the skin, microneedling stimulates regeneration.
  • Microneedling + mesotherapy: in the same visit — microneedling opens channels in the skin so the cocktail absorbs better.
  • PRP + microneedling: the classic “vampire facial”.

Before the procedure

  • 1 week before: avoid retinoids and hydroquinone.
  • 3 days before: no facial peel or waxing.
  • 24 h before: do not drink alcohol.
  • On the day of the visit: come without makeup.

After the procedure

  • Day 0–1: skin is red, mild swelling. Use nourishing creams (niacinamide, hyaluronic acid).
  • Day 2–4: mild flaking. Wash with a gentle cleanser.
  • Day 5+: skin tone improves, glow appears.
  • SPF 50+ mandatory for 2 weeks.
  • Avoid the sauna, tanning bed and intense exercise for 48 h.

Who it is not suitable for

  • Active acne (with a severe breakout).
  • Active herpes phase.
  • Blood clotting disorders, anticoagulants.
  • Pregnant and breastfeeding women.
  • Isotretinoin treatment within the last 6 months.

  • Ask for the device name and model — ageing dermapen devices do not deliver results.

What is the difference between microneedling and mesotherapy?

Microneedling (dermapen) is a mechanical procedure — the needles create micro-punctures that stimulate collagen. Mesotherapy is an injection-based procedure in which a cocktail of vitamins, hyaluronic acid or peptides is injected into the skin. The two are often combined.

How many sessions are needed?

For a visible effect, 3–6 sessions at 2–4 week intervals. To heal acne scars, 6–10 sessions. For maintenance, 1 session per quarter.

How much does it cost in Estonia?

Microneedling is 70–140 € per visit in Tallinn. Mesotherapy is 80–180 €. A combination (PRP or peptides) is 120–220 €. Packages of 4–6 sessions are usually 15–20% cheaper.

Is it painful?

Mild discomfort — a numbing cream is applied 20–30 minutes beforehand. During the procedure it feels like “needles rolling”. In the recovery period there is mild redness and dryness for 1–2 days.
